Memorial Giving
A donation given in lieu of flowers is a wonderful way to
celebrate the life of someone who has passed away. These donations
are applied directly to equipment needs to help patients at
the hospital.
When you give a memorial gift
to Port Perry Hospital Foundation, we send a note to the next
of kin acknowledging your thoughtful contribution in memory
of their loved one. (The amount of your gift is kept strictly
confidential.) We then forward a charitable tax receipt to
you.
When sending a memorial donation,
print and fill in the donation form, including the name of
the deceased, as well as the name and address of the next
of kin.

In Honour Gifts
People give in-honour donations to mark a special occasion,
such as a wedding or anniversary, or to say “thank you”
to someone. We acknowledge these gifts by sending a card to
the celebrant in your name, and you will receive a charitable
tax receipt. As with memorial giving, the amount of your gift
is not disclosed.

people who have had their lives touched by doctors and staff
at Port Perry Hospital decide to leave donations to the Foundation
as part of their estate planning. Legacy giving is a thoughtful
way to support the hospital you care about either now, or after
you’re gone.
Legacy gifts can include bequests,
stocks and bonds, life insurance, charitable trusts, retirement
plans and property.
Giving a legacy gift requires
thorough planning, involving family members and financial professionals.
Timing and taxes have a significant affect on the important
decision to make a legacy gift.

Bequests:
A charitable bequest is a gift specified
in your will.
You
can designate a certain amount of money, a particular
asset, a part of your estate, or your entire estate to
Port Perry Hospital Foundation. We recommend that you
speak to a lawyer to ensure your wishes are set out clearly
in your will.
Specific
Bequests are when you designate a specific dollar amount
or piece of property. Residual Bequests are when you donate
all or a portion of your estate to the beneficiary after
all your debts, taxes, expenses and other bequests have
been paid.

Stocks
and Bonds: Giving
publicly-listed securities, like stocks, bonds or mutual
funds is an excellent way to support Lakeridge Health
Port Perry and it provides great tax advantages to the
donor.
Call
the Foundation at 905.985.7321 ext. 5581 for the information
you will need to transfer your securities. Then contact
your broker or financial advisor to discuss making a gift
of securities to your local hospital. Your broker will
contact Port Perry Hospital Foundation to arrange the
transfer. Securities are equally effective when given
today or in the future. You will receive a charitable
gift receipt for your donation and save the capital gains
tax. When you transfer securities to the Port Perry Hospital
Foundation you receive a donation receipt for the closing
price on the date of the transfer without activating the
capital gains tax. |

| Life
Insurance: There are a few ways to use life
insurance to make a gift to Lakeridge Health Port Perry.
You can give ownership of a new or an existing policy
to the Port Perry Hospital Foundation, or you can name
the Foundation as the beneficiary of the policy. There
are different tax consequences that benefit different
stages of life with each option. |

| Retirement
Plans: You can appoint the Port Perry Hospital
Foundation as the beneficiary of your RRSP or RRIF, or
make a donation in your will equal to the proceeds from
your retirement plan. The charitable donation tax credit
has been extended to include the proceeds from RRSPs and
RRIFs. If you name the Port Perry Hospital Foundation
as the beneficiary of your retirement plan the proceeds
may be transferred directly to the Foundation, which could
help protect the gift from other estate costs. |

| Trust:
You can set up a trust fund with your gift. The trust
will allow you to assign interest income to your loved
ones during their lifetimes. Upon your death and that
of any beneficiaries the capital is transferred to the
Foundation. |

| Property:
You can donate real estate, shares in privately-held corporations,
royalty interest, works of art, jewellery and other property
to Port Perry Hospital Foundation. |
