Another way to give – Support the Dragon Boat Festival:
Support the Dragon Boat Festival here, by registering as a participant or by supporting your Dragon Boat team or paddler.
A donation, sometimes given in lieu of flowers, is a wonderful way to celebrate the life of someone who has passed away. Memorial donations help to ensure updated medical equipment and other high priority needs can be met in the delivery of health care for our community.
When you give a memorial gift to Port Perry Hospital Foundation, we send a note to the next of kin acknowledging your thoughtful contribution in memory of their loved one. (The amount of your gift is kept strictly confidential.) We then forward a charitable tax receipt to you.
When sending a memorial donation, print and fill in the donation form, including the name of the deceased, as well as the name and address of the next of kin.
Mail your gift to:
Port Perry Hospital Foundation, 451 Paxton Street, Port Perry, ON L9L 1L9
In Honour Gifts
In-Honour donations mark a special occasion, such as a wedding or anniversary, or to say “thank you” to someone. We acknowledge these gifts by sending a card to the celebrant in your name, and you will receive a charitable tax receipt. As with memorial giving, the amount of your gift is not disclosed.
Planned and Legacy Giving
Having had your life or a loved one’s life touched by care at Port Perry Hospital or simply wishing to support your community in a lasting and meaningful way is reason to consider a planned or legacy gift.
Planned gifts include bequests, gifts of securities (stock gifts), life insurance, charitable trusts, retirement income, and property.
A charitable bequest is a gift specified in your will. You can designate a certain amount of money, a particular asset, part or all of your estate to Port Perry Hospital Foundation. We recommend that you speak to your lawyer to ensure your wishes are set out clearly in your will.
As well, we encourage you to call the Foundation to ensure we can use your donation as intended. Plus, when you let the Foundation know, it allows us to properly say thank you now, today, for the wonderful legacy you plan to leave in the future.

Gifts of Securities
Giving publicly-listed securities, like stocks, bonds and mutual funds, is an excellent and tax-savvy way to provide support.
Contact Foundation CEO, Rachel Agnoluzzi at 905-985-8860 for the information you will need to transfer your gift of securities and to provide information required for receipting purposes. Your broker or financial advisor can advise you and help you facilitate the transfer.
Your gift will be eligible for a charitable tax receipt and, because capital gains on securities transferred to a charity are tax-exempt, there is potential for significant further advantage to you as the donor.
There are a few ways to use life insurance to make a gift to Lakeridge Health Port Perry. You can give ownership of a new or an existing policy to the Port Perry Hospital Foundation, or you can name the Foundation as the beneficiary of the policy. There are different tax consequences that benefit different stages of life with each option.
Your insurance representative can help you set this up, but please remember to let the Foundation know of your plans to ensure we can use your gift in keeping with your wishes
You can appoint the Port Perry Hospital Foundation as the beneficiary of your RRSP or RRIF, or make a donation in your will equal to the proceeds from your retirement plan. The charitable donation tax credit has been extended to include the proceeds from RRSPs and RRIFs. If you name the Port Perry Hospital Foundation as the beneficiary of your retirement plan the proceeds may be transferred directly to the Foundation, which could help protect the gift from other estate costs.
You may wish to consider a donation of real estate, shares in privately held corporations, royalty interest, art, jewelry, etc. Please contact Foundation CEO, Rachel Agnoluzzi at 905-985-8860 in advance of a donation of property to ensure we can take it for its intended purpose.
The information herein is available to assist you. However, we encourage you to seek any and all legal and financial advice necessary while considering a planned or legacy gift to the Foundation.
