Memorial Giving

A donation is a meaningful and generous way to honour the life of someone who has passed away.

When you give a memorial gift to Port Perry Hospital Foundation, we shall send a note to the next of kin notifying them of your thoughtful gift in memory of their loved one. The amount of your gift is kept confidential and only the combined total of all memorial donations will be shared with the family.

Your donation is tax receiptable in accordance with CRA guidelines.

To make your donation:

• Complete the attached Donation Form (PDF) and mail it along with your gift to:

• Call the Foundation office at 905-985-7321 ext 45580, or
• Give online at Give Today!  You will have the choice of sending an eCard directly to the next of kin or instruct us to send a card in the mail on your behalf.
